How to Build a U.S. Network From Zero: A Cold Outreach Playbook for IMGs

One of the biggest myths I hear from international medical graduates is this:

"I can't network because I don't know anyone in the United States."

If that sounds familiar, I want to challenge that belief.

You don't need family connections.

You don't need a famous medical school.

You don't need someone to introduce you to residency programs.

You simply need a strategy.

Networking is one of the most valuable investments you can make during your residency journey, and today I want to show you how to start—even if you're beginning from zero.

Why Networking Matters

When most IMGs think about networking, they picture asking someone for a clinical rotation or a letter of recommendation.

That's only a small part of it.

Real networking creates relationships that can lead to:

  • U.S. clinical experience

  • Strong letters of recommendation

  • Research collaborations

  • Mentorship

  • Professional sponsorship

  • Career guidance

  • Residency opportunities

The strongest applications are often supported by people who know you—not just your CV.

Step 1: Change Your Mindset

The biggest mistake I see is approaching networking with only one goal:

"What can this person do for me?"

Instead, ask yourself:

"How can I build a genuine professional relationship?"

People can sense when they're being treated like a stepping stone.

Instead of chasing opportunities, build authentic connections.

Opportunities naturally follow.

Step 2: Find the Right People

Don't automatically email the program director.

Instead, identify physicians with something in common.

Maybe they:

  • Share your clinical interests

  • Have a similar background

  • Graduated from your medical school

  • Conduct research you're interested in

  • Are current residents

  • Are faculty members involved in education

The more personalized your outreach is, the better.

Step 3: Personalize Every Cold Email

Generic emails rarely work.

Your introduction should immediately create connection.

Instead of introducing yourself only as an IMG, introduce yourself through your professional identity.

Mention your current clinical role.

Reference their research.

Explain why you're specifically reaching out to them.

Show that you've done your homework.

Remember, people respond to genuine interest—not copied templates.

Step 4: End With a Simple Question

One mistake many IMGs make is asking for too much too soon.

Instead of immediately requesting a rotation, ask something simple.

For example:

  • Would you be open to answering a few questions about your specialty?

  • May I ask about your career path?

  • Would you be willing to share advice for someone pursuing internal medicine?

Simple questions lower the barrier to responding and often begin meaningful conversations.

Step 5: Join Your Specialty Organizations

Every specialty has organizations dedicated to education and professional development.

Become a member.

Attend webinars.

Participate in educational events.

Introduce yourself.

These organizations provide access to mentors who genuinely enjoy helping future physicians.

Step 6: Continue the Relationship

Networking doesn't stop after one email.

Stay connected.

Ask how you can help.

Volunteer for projects.

Offer assistance with research.

Express gratitude.

Professional relationships grow over time.

It's about building trust.

It's about showing curiosity, professionalism, and consistency.
