Rural Programs Need IMGs. Here Is How to Show Them.
Rural programs are not doing IMGs a favor by taking us. They need us.
I did my residency at the Rural Training Track of the University of Kentucky. Then I practiced in Appalachia for six years. I have seen this from the inside.
Why rural programs value IMGs
If you practiced medicine in a resource-limited setting in your home country, you already know how to function without subspecialty backup. You have managed complex patients when there was no one to call. Rural programs recruit for exactly that experience.
The J-1 waiver system reflects the same reality. It exists to bring physicians into shortage areas, and it has created a long pipeline of IMGs into rural medicine.
What each setting preference signals
You can and should apply to urban programs if you want them. But if you only click urban by default, you are competing on US graduates' home turf.
Declare it if it is genuine
If rural training is truly on the table for you, select it and use the description box to explain why. Name the resource-limited settings you have worked in and what you learned there.
Do not write it like an apology. Write it like the qualification it is.
