Is matching dermatology as an IMG really as rare as people say?

Yes, it is rare, and that is exactly why Dr. Ramirez’s story matters. Dermatology is one of the most competitive specialties in the Match, and IMGs are often told not to even try. But Dr. Ramirez chose not to let that narrative decide her future. Instead, she focused on strengthening every part of her application until she could stand confidently in the room with anyone else applying.

What helped Dr. Ramirez become a serious dermatology applicant?

One of the biggest turning points in her journey was committing to a research-focused path. Dr. Ramirez knew that if she wanted to compete in dermatology, she needed more than interest alone. She needed a stronger academic profile, deeper specialty exposure, and mentors who could advocate for her. That is what led her to pursue a postdoctoral research position in the United States.

Why was a postdoctoral research position such a big deal for her application?

For Dr. Ramirez, the postdoc was not just a line on her CV. It gave her access to dermatology-specific research, publications, mentorship, networking opportunities, and strong letters of recommendation. It also helped her become known within the field. In a specialty like dermatology, where relationships and reputation matter, that kind of visibility can make a major difference.

How did Dr. Ramirez find research opportunities in the first place?

She did not wait for opportunities to come to her. She went looking for them. Dr. Ramirez used social media, conference networking, and a large amount of cold emailing to find open doors. She searched faculty pages, reviewed people’s research interests, and reached out directly. Her story is a strong reminder that many opportunities start with one email, one introduction, or one moment of courage.

Did she already know she wanted dermatology from the beginning?

Not immediately. Like many IMGs, she explored different specialties before fully committing. Through observerships and clinical exposure in the U.S., she narrowed her interests and eventually chose dermatology. That process of exploration helped her make a more confident decision and also gave her more insight into how to shape her application intentionally.

What role did U.S. clinical experience play in her journey?

It helped more than people may realize. Dr. Ramirez completed observerships while still in medical school and used those opportunities to learn the U.S. system, build relationships, and stay connected with mentors over time. Even though some of those experiences were not fully hands-on, she still used them wisely. She stayed engaged, asked thoughtful questions, and made the most of every opportunity she had.

What were her USMLE scores, and did they matter?

Yes, they mattered. Dr. Ramirez passed Step 1 and scored 255 on Step 2, which placed her in a competitive range for dermatology. She is very clear, though, that scores are only one part of the application. Strong scores help, but in a field like dermatology, scores alone are not enough. Research, relationships, letters, and program familiarity all matter too.

Did Dr. Ramirez need to know programs personally before applying?

That was one of the biggest patterns she noticed. Many IMGs who match dermatology already have some form of prior connection to the programs where they interview or match. That connection might come through a mentor, a research year, an away rotation, a conference interaction, or collaborative work. Dr. Ramirez applied strategically to programs where she had built real touchpoints, not just places she liked on paper.

What can other IMGs learn from Dr. Ramirez’s story?

Her journey shows that competitive specialties require more than hope. They require strategy. If you are an IMG pursuing something highly competitive, you may need to think several steps ahead. That might mean building research experience early, seeking out mentors, creating meaningful professional relationships, and being willing to do the extra work that makes your application impossible to overlook.

What mindset helped Dr. Ramirez keep going?

One of the most powerful things she shared was this: do everything with love. She talked about staying curious, putting her full heart into each opportunity, and trusting that even small experiences could serve a bigger purpose later. That mindset helped her keep showing up, even when the path ahead was uncertain.

So, is dermatology possible for IMGs or not?


Dr. Ramirez’s story makes one thing clear: yes, it is possible. It may be rare. It may be difficult. It may require more strategy and sacrifice than people realize. But impossible? No. And sometimes seeing one person do it is exactly what another person needs in order to finally believe they can too.
