Dear IMG Roadmap students,

Welcome! The IMG Roadmap Membership is a monthly, longitudinal learning program, and I'm your host, Dr. Lum. My hope is to help you navigate the complex process of getting into residency with greater ease and confidence.

Complete Your Intake Form

So I can serve you as well as possible, please fill out your membership intake form using the link below.

Review & Sign the Membership Agreement

This is an agreement between you and Dr. Nina Lum. Please read it in full and sign your own copy.

How to sign

  1. Click the agreement link below.

  2. On the page that opens, click the "Fill a copy" button in the upper-right corner.

  3. Check the box "I agree to use electronic records and signatures," then click "Let's do this."

  4. DocHub will guide you to each field you need to complete and sign.

  5. When you're finished, click "Finalize" to submit your signed document.
